*These fees apply also to complicated telephone consults, home visits, admissions, police calls, listening to tapes, looking at records, writing reports, etc. that take over 5 to 10 minutes. These may not be reimbursable. Most private insurance companies cover 50 to 100% of your bills. I will help you with forms during your sessions.


CONFIDENTIALITY

In general, all communications between patient and doctor are privileged and confidential.

Exceptions: For insurance and/or disability purposes, information is often required about your diagnosis, nature of disability, prognosis, medications, treatment plan, etc.

In the case of imminent risk of physical harm to yourself or others (suicidal or homicidal plans, inability to function), I may need to involve family members or authorities to ensure safety. Confidentiality does not apply if you are in the hospital or hospital emergency room for risk assessment.


EMERGENCIES

For emergencies, if unable to reach me immediately, go to the hospital emergency room closest to you by any appropriate means (eg. driving yourself, having a family member or friend drive you, or calling 911 for an ambulance), depending on the nature and severity of your condition.


EVENINGS AND WEEKENDS

For urgent questions that need to be addressed before the next business hours, try to reach me at my home office in Rochester. You may have to leave a message and I will return your call as soon as possible.


HOSPITAL ADMISSION

The need for inpatient care occurs rarely in my practice. If and when the need arises, I will help facilitate admission to a hospital. I will not be the inpatient admitting/attending psychiatrist, but I will coordinate your care with your in-patient psychiatrist and with the hospital staff, who will call me for information, notify me of significant events and arrange your follow-up appointment.


MEDICATIONS

Normally, I schedule medication-only visits every three months. If you come more often for therapy as well, please try to be aware of the expiration dates of your prescriptions, so that you can ask for them at the appropriate session. If we don't already have an appointment, call two to three weeks before your medications run out, or preferably, at the time you get your last refill, to schedule a follow-up appointment.
